Care Co-ordinator (Non-Clinical) – Trowbridge Primary Care Network
Based at: Lovemead Group Practice, Roundstone Surgery, Trowbridge
Hours: 20 per week – negotiable

About the Role
This role will focus on supporting our local nursing homes and learning disability (LD) homes, ensuring effective communication, coordination, and continuity of care for residents.
Responsibilities
* Liaising regularly with nursing homes and LD homes to arrange and support routine visits and reviews
* Organising clinic and ward round schedules for visiting GPs
* Coordinating learning disability health checks and ensuring timely completion of assessments
* Supporting vaccination programme planning and scheduling for care home and LD home residents
* Maintaining accurate records and ensuring information is shared appropriately across the multidisciplinary team
* Providing general administrative support to the PCN care home and LD services
About You
We are looking for someone who is:
* Highly organised with excellent attention to detail
* Able to work independently as well as part of a wider multidisciplinary team
* A confident communicator, able to liaise professionally with care home staff, clinicians, and patients’ families
Essential Skills and Experience
* Proven administrative experience in a busy environment
* Proficient in Microsoft Office (Word, Excel, Outlook)
* Excellent communication and organisational skills
Desirable
* Previous experience working in primary care or health and social care settings
