Specialist Community Neuro rehab Physiotherapist

We are Trafford Local Care Organisation - an exciting public sector organisation bringing together NHS community health services and adult social care services in Trafford. We're driven by the aim of improving the health of local people in Trafford by working across organisational boundaries.

Our community healthcare teams in adults and children's services are employed by Manchester University NHS Foundation Trust and deployed to us, linking closely with our colleagues in Manchester Local Care Organisation. Our teams work as one and together we will make a massive difference to the health of people in Trafford.

This is an exciting opportunity for a dynamic, professional and enthusiastic team member to work autonomously with a stroke and neuro-rehabilitation caseload. You will be joining a well-established MDT which offers community stroke and neurological rehabilitation pathways. We provide services in clients' homes and other community settings.

You will take an active role in team and service meetings so contributing to the positive development of the team, and assist in the supervision of other Physiotherapists and support workers.


Main duties of the job

To provide a specialist patient centred physiotherapy service to those who are referred to the community stroke and neuro rehabilitation service.

To undertake the assessment and clinical diagnosis of neurologically impaired patients using investigative and analytical skills; formulating an individual management and/or treatment programme according to the patients goals using advanced clinical reasoning and a range of treatment skills, including knowledge of alternative treatment options.

To undertake the continuous assessment and evaluation of treatment programmes to ensure maximum patient benefit, and modify them according to the patient's needs.

To assess and manage clinical risk and accept clinical responsibility as an autonomous practitioner for a designated caseload of patients, and organise this efficiently and effectively with regard to clinical priorities and use of time.

To utilise complex communication skills to be able to overcome barriers to communication.

Provide advice and education to patients and carers in conjunction with the multi-disciplinary team and signpost to appropriate agencies and charities.

To be aware of, and comply with, organisational and departmental policies and procedures and to be involved in reviewing and up‑dating as appropriate.

To maintain own continuing professional development and develop current knowledge of evidence‑based practice.

Person Specification

* Diploma/Degree in Physiotherapy
* HCPC Registration
* Evidence of post graduate study in the field of Neuro‑Rehabilitation
* Student Accreditation
* Appropriate relevant post graduate experience working in a community setting
* Evidence of working in an NHS multi‑disciplinary team
* Evidence of working with clients with specialist stroke and neuro rehabilitation needs
* Supervision of junior staff/ students
* Clinical Audit and standard setting
* Knowledge and ability to implement current best practice in neuro rehabilitation
* Evidence of specialist treatment applications
* Evidence of working with people with multiple needs
* Communication skills verbally and in writing to address physical/ psychological and socio‑economic needs
* Demonstrate ability to manage time effectively
* Ability to undertake appropriate therapy assessments relevant to the clinical speciality
* Able to use clinical reasoning to make a therapy diagnosis and formulate treatment plans
* Knowledge of issues relating to Clinical Governance
* Awareness and understanding of specialism specific local and national guidelines
* Self‑reliant and self‑motivated and be able to seek support and help when appropriate
* Commitment to patient centred non‑discriminatory practice
* Reflect and critically appraise one's own performance
* Current UK Valid Driving License
* Access to a car
* Evidence of professional networking in neuro rehabilitation
* Evidence of service development
* Attendance on leadership course
* Experienced in the application of functional electrical stimulation
* Experienced in the assessment and provision of postural management systems

£39,959 to £48,117 a year Per annum (pro rata)
