About the Role
Somerset is seeking an experienced and highly skilled Advanced Practitioner Social Worker to join our Adult Social Care service.
This is a key leadership role within a neighbourhood or locality-based team, providing professional oversight, practice leadership, and operational support to ensure high-quality social work delivery across Adult Social Care.
You will play a vital role in enabling adults to remain independent for as long as possible, supporting people to access timely, strengths-based care and ensuring that services are joined-up across health, social care, and community networks.
The focus of this role is to promote independence, prevent unnecessary hospital admissions, and support safe and timely discharges—ensuring that individuals remain at the centre of all decision-making and care planning.
Key Responsibilities
- Provide professional leadership and practice oversight within the team
- Support the Service Manager in delivering operational performance and service quality
- Undertake oversight of complex and high-risk cases, including joint work and case holding where required
- Lead and contribute to safeguarding adults processes, ensuring timely and effective responses
- Provide high-quality supervision and appraisal to qualified social workers
- Coach, mentor, and support staff to develop professional practice and confidence
- Ensure timely, strengths-based assessments, care planning, and reviews in line with the Care Act 2014
- Chair and contribute to complex meetings, including safeguarding and best interest processes
- Support legal processes, including Court of Protection applications where required
- Undertake audits and contribute to service quality assurance and improvement
- Analyse complex information and produce clear, evidence-based professional recommendations
- Support performance monitoring and service improvement initiatives
- Contribute to workforce development, training, and induction of new staff
- Use delegated financial authority responsibly, including personal budget decision-making
- Support effective budget awareness and demand management
- Provide professional input into court proceedings and legal reports when required
