We seek a Nurse with experience, skills and passion to join our Specialist Community Nurse team for young people with an Intellectual Disability, providing advice and support to professionals, parents and carers of children aged 5 to 19 who have a diagnosed Intellectual Disability. You will use your knowledge and experience of complex health conditions and have a flexible and adaptable approach to your workload. You will support the Single point of Entry referral meetings and manage a clinical caseload.

Our Children's Nursing Team work alongside CAMHS colleagues in Intellectual Disability Service and Complex Behaviour Service.

We are looking for a Nurse with the skills and confidence to use effective time management, diary management and an ability to prioritise clinical demands, you will receive line management/supervision and clinical supervision and comprehensive training, to support you to develop in your role.

We particularly welcome applications from Nurses with specialist interest, skills, knowledge and understanding of behavioural support approaches, sleep management and Neurodevelopmental conditions.

The role includes group work for parents and carers, alongside assessment and direct work.


Main duties of the job

To provide child-centred holistic care.

To review the health needs of children with I.D and complex presentations.

Be a participant in the transition of pupil's care to other service areas.

Prioritise the implementation and evaluation of health care plans and refer to other professionals as required.

To signpost to health education programs to meet the health needs of the defined population in line with national / local priorities & strategies.

To keep accurate, and current records

To development and deliver training packages.

Provide advice and support in the development of health promotion / education strategies.

Support and enhance in the learning environment for pre and post registered students.

Participate in the clinical supervision program.

Comply with Trust policies and procedures.

Manage a defined caseload, knowing when appropriate to discharge.

Attend SPOE and referral management within the team.

Communication sharing regarding transition of children with complex needs.

Working in partnership with Universal services - school nurses and health visitors to ensure all health needs are met and roles and responsibilities are clear.

To be familiar with Health and Safety legislation and to be responsible for reporting patient safety or staff incidents or areas of risk.

To contribute to the effective implementation of safeguarding children through the accessing of training and safeguarding supervision in line with Local Safeguarding Board procedures.

Job responsibilities

The post holder will be working with children aged 5 -19 who have a diagnosed intellectual disability and their families. The post holder must become competent in understanding the management of children and young people with an I.D. and complex health issues and the possible impact upon the wider family.

The post holder will be responsible for the supervision of other staff within the team. The post holder will be required to implement and evaluate individualised care plans, support parents/ carers with education, strategies to self-manage and signpost. The post holder will be contributing to referral management and attend Single point of entry meetings (SPOE), comprehensive assessment processors and carry a defined caseload. The role will include overseeing the management of children with a confirmed I.D supporting with planned clinics for sleep and behaviour support. The role will require someone who is able to show real empathy to complex situations whilst encouraging resilience to the parents and carers who are receiving direct support.

We are looking for someone with excellent communication skills, including verbal or Makaton with a real interest and knowledge of Intellectual disabilities and associated health related concerns. A commitment to advocating, signposting, and working in partnership with other agencies including local communities is essential in order to deliver the best possible outcomes to this cohort of children, young people and their families.

Regular liaison will be available by the operational/ clinical leads, Complex Behaviour service and general supervision will be provided by the Leads within the team.

Person Specification


Qualifications

* Qualified Nurse in Children's, adult, Learning Disabilities or Mental health
* Evidence of Continuing Professional Development
* Qualification in a therapeutic modality
* BABCP registration
* Post graduate qualification in Child Mental Health
* Registered AMHP


Experience

* Experience of multi-agency working.
* Experience of working in a team and working autonomously
* Experience of carrying out audits
* Experience of waiting list management
* Knowledge & or experience of Autism, learning disabilities and associated conditions
* Community Experience
* Experience of supervising staff
* System one experience
* Experience of working with disabled children and young people
* Experience of working with Intellectual disabilities


Skills

* Carry out intense conversations with service users regarding packages of care and evaluating progress
* Keeping up to date and detailed health records that fully reflect your decision making and are in line with the Trust guidelines.
* Committed to promoting equality, diversity and inclusion for children with I.D &/ or complex Behaviour difficulties
* Ability to organise and prioritise workload and work without direct supervision, accessing supervision appropriately.
* Skills & experiences in assessing needs, planning care and reviewing treatment. Effectively manage and report risk


Knowledge

* Knowledge of local and national Policies and Nice Guidance
* Knowledge of sleep difficulties and sleep strategies
