Your role will be pivotal, as it involves local visits to Leicester residents’ homes, hospitals, and Discharge to Assess facilities across the city.

What you will do
* Collaborate with multidisciplinary teams to ensure effective discharge planning and reablement for clients.
* Conduct assessments in clients’ homes, hospitals, and city facilities to tailor reablement programmes that promote independence.
* Develop and implement personalised care plans in partnership with clients, family members, and healthcare professionals.
* Monitor and review clients’ progress, adapting care plans as necessary to meet their evolving needs.
* Maintain accurate and timely records of all interactions and interventions in line with the city’s policies and procedures.
* Provide expert advice and support to clients and their families, helping them navigate the healthcare system effectively.
