Main duties of the job
Our service model is built on locality‑based working, enabling tailored healthcare delivery aligned with the unique needs of our diverse populations. BCHC is proud to support multi‑disciplinary frailty pathways across neighbourhoods, blending inpatient, community, and virtual care models. Our teams work closely with local GPs, social care, mental health services, and acute providers to ensure seamless and holistic care.

Person Specification
Essential
 * Broad experience in Geriatric Medicine /Care of Older Person or General Internal Medicine
 * Excellent communication and interpersonal skills
 * Experience in teaching medical students and Resident doctors
 * Flexibility and adaptability
Desirable
 * Experience in community or intermediate care settings
 * Understanding of NHS structures and community care pathways
 * Interest or experience in research or service evaluation
More Essential requirements
 * Full MRCP, MRCGP or equivalent
 * Entry on GMC Specialist Register (Geriatrics & GIM) or within 6 months of CCT at time of interview
 * Experience in community or intermediate care settings
 * Ability to manage complex patients in a community setting
 * Leadership and decision‑making ability
