Overview
We are seeking to appoint an enthusiastic, motivated and reliable Community Health Care Support Worker to deliver a range of quality care services to patients visited by the Care Community Team based at Chowley Oak, Tattenhall. The team visits patients of the Rural Alliance Care Community.
Responsibilities
* Work in patients' homes as part of a friendly team; carry out tasks such as simple dressings, catheter care, personal care and recording blood sugars.
* Provide care in the final days/weeks of life; lone working; support to prevent acute hospital admission and to facilitate discharge from hospital.
* Travel across the Western Cheshire footprint; access to transport for work purposes is essential.
Role details
This role is part time – 30 hours per week.
Pay and employment information
The Trust currently supports the Real Living Wage (RLW) hourly rate as set by the Living Wage Federation and will apply a pay supplement if the national A4C hourly rate falls below this.

Induction and start date
All new starters will commence in post on one fixed start date each month, in line with our Prepare to Care (P2C) induction programme date. P2C offers up to 3 weeks of training to support you to meet the induction / initial training requirements of the role.
