Overview
An exciting opportunity for a support worker/trainee to join the assertive outreach and recovery team which supports patients in the community with their mental health recovery. The post involves working with people in their homes, in community clinics, and providing in-reach to patients admitted to hospital to support discharge home. The post holder will work as part of the multi-disciplinary team delivering high quality, patient-focused care in a community setting and promoting recovery and wellbeing, with some physical health interventions as part of care.
Responsibilities
* Work with individuals in community settings, in their homes and in hospital in-reach to support discharge planning.
* Collaborate with the multidisciplinary team to promote recovery for people with complex needs, including psychosis, substance use challenges, housing and social/emotional needs.
* Promote patients’ independence and deliver excellent services to support recovery, wellbeing and social participation.
* Provide some physical health interventions as part of holistic care.
* Work alongside care coordinators and lead professionals to achieve patient-centred, recovery-focused care.
* Aim to enable individuals to reach the highest possible level of independence and quality of life.
* Demonstrate compassion, inclusivity, and commitment to delivering excellent care within a supportive team environment.
Qualifications and requirements
* Experience working within substance use services or with individuals experiencing substance use challenges is desirable but not required.
* Must have completed the Diploma in Health and Social Care Level 2 or equivalent.
* Literacy and Numeracy Level 2 or equivalent.
* Must complete the Trust Healthcare Assistant competencies within their probationary period (first 6 months in role).
* If the required qualifications are not held, candidates will be expected to enrol on the Healthcare Assistant Framework, which includes acquisition of Level 2 Diploma Health and Social Care, Level 2 in Literacy and Numeracy or equivalent, and achievement of competencies. A maximum of 2 years is allowed to complete this.

Right to Work in the UK is required to be successfully appointed to this role. Please note, this role does not meet the required eligibility criteria for sponsorship under the Skilled Worker visa and therefore the Trust is unable to offer sponsorship for this particular role.
