If so, we have an exciting opportunity for a Band 5 Mental health Practitioner to work within the Easington Community Hub team.

You will be an active member of our multi-disciplinary team (MDT), working as a Key Worker, with a caseload which will be challenging at times, as well as rewarding. We will offer you the opportunity to work with a highly skilled and experienced MDT providing support, development opportunities and supervision for you, as well as excellent standards of care for our service users and carers.

We are an established team working in peoples own homes and other care environments.

Please note that the standard working hours for this role are Monday to Friday 9am to 5pm

Main duties of the job

It is therefore important that the successful candidate is a flexible team player and holds excellent interpersonal and communication skills to meet the needs of the patients, carers, families and the partners with whom we work.

As a Community Practitioner you will have a genuine interest in collaborative working with adults with sever and enduring mental health needs.

As a registered professional this is an ideal opportunity for someone who wants to develop their community clinical skills. As a core member of the team, you will also be responsible for your own caseload, providing ongoing assessment, care co-ordination as a Key Worker and delivering evidence based intervention in the community. You will demonstrate excellent assessment skills, interpersonal, and team working skills and be able to work flexibly and collaboratively with patients, families/ carers and a range of statutory organisations.

You will have an enthusiastic approach towards your own personal and professional development. In return, you will receive regular appraisal, clinical, management and caseload supervision and will benefit from the support of an experienced and committed multi-disciplinary leadership team, including the Team Manager and Advanced Practitioners.

Qualifications

Essential

1. Current professional registration with an approved body
2. Evidence of continuing professional development relevant to post
3. For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education or willingness to work towards. Qualification must be achieved within agreed timescale.
4. Trained Clinical Supervisor (within agreed timescale).
5. Key skills in literacy, numeracy and ITQ level 2 (or equivalent)

6. Post-graduate qualification in a relevant area

Experience

7. Working with people with mental ill-health

8. Working in a person centred therapeutic organisation
