Job overview An exciting and rare opportunity to join a motivated and resilient nursing team working with Criminal Justice Mental Health and specifically the Integrated Offender Management Referrals pilot pathway who require an assertive and resilient approach. The objectives are to positively; improve health outcomes, reduce offending and improve the lifestyles of those experiencing mental health concerns resulting in engagement with the criminal Justice system. Advert The detail of the role is included in detail of the Job description and Service specification but includes: Networking with: Criminal Justice partners including police, probation, prison, approved premises Families, carers and those identified as significant to referrals Health partners including trust services, GP's, primary care partners and substance services Voluntary, community and social enterprise partners Provision of: Mental Health and Risk Assessment Support and Treatment of Mental Health Diagnosis Practical, psycho-educational and psycho-social support of referrals Formulation and planning of care plans to support referrals Team work: Working as part of a Multi-disciplinary network Sharing Risk and appropriate documentation completion Supporting Students and colleagues in practice Ensuring training standards are maintained Engage with the team and partners to innovate and develop as required It is essential to be able to travel across the Black Country in a timely manner therefore driving with business insurance is essential. A Police Check will also be required. This will require a lot of driving across the Black Country Borough, therefore business insurance will be essential. This service offers face to face appointments, across criminal justice, health and community venues in the Black Country. It works in an assertive manner to achieve the service objectives and faces capacity and demand challenges daily, therefore staff need to be resilient and motivated for this intense service remit. Standards and checks · Clinical posts up to and including Band 4 are expected to hold the Care Certificate.
