Overview
Due to recent developments, Urgent Care are looking to recruit a social worker who will provide social work support, strength‑based assessments, tailored support planning and reviews in line with the Care Act 2014 for adults across Trafford. Responsibilities
You will need to hold a relevant social work qualification and be registered with Social Work England. You will undertake strength‑based assessments and deliver person‑centred support and outcomes for adults with care and support needs. You must promote independence, recognise potential for change and enable people to maximise their strengths. You will provide an effective response to safeguarding enquiries in line with the Care Act 2014 and local policy and procedures.
You will work as part of a multidisciplinary team and autonomously, familiarising yourself with the Mental Capacity Act 2005 and professionally challenging to ensure best outcomes for residents. You must possess good communication skills and be able to work proactively with a range of adults, applying a clear understanding of the legislative framework for adult social care.
Qualifications
Relevant social work qualification and registration with Social Work England.
