🚀 Transition Social Worker (Children ➜ Adults) – Learning Disability & Mental Health
Long-term temp assignment · 2 years · 36 hrs pw · Monday–Friday (occasional Sat) · Wilshaw House, Tameside

What you’ll do
4. Assess & plan: strengths-based Care Act assessments, Mental Capacity decisions and outcome-focused support plans.
5. Coordinate care: commission services, monitor delivery and review changing needs for young people with learning disabilities, autism and/or mental-health conditions.
6. Collaborate: work hand-in-hand with CAMHS, SEND, schools, health and housing so every transition feels seamless.
7. Safeguard & evidence: prepare court statements (DoLS, COP DOL, s21a) and give confident evidence when required.
8. Champion carers: complete statutory Carer assessments and link families to the right support.
What you’ll bring
9. Social Work degree/DipSW & Social Work England registration.
10. Proven practice with young people who have learning disabilities and/or mental-health needs—ideally around transition.
11. Up-to-date knowledge of the Care Act, MCA & SEND/EHCP frameworks.
12. Strong assessment, report-writing and partnership skills, plus a full UK driving licence & car.
