We are a specialist team made up of Physiotherapists, Nursesand Support workers delivering Pulmonary Rehabilitation and respiratory care topatients living in their own homes.
We have close links with primary and secondary care workingclosely with colleagues to deliver high quality care for our patients.
Key ongoing developments are the expansion of the PulmonaryRehabilitation Service, and developing partnership working with virtual wardsto help prevent hospital admissions for the Respiratory population we serve.

Main duties of the job
The Band 6 Physio within the Community Respiratory team willundertake a leading role in the planning, coordination, delivery and evaluationof high-quality assessment at Pulmonary rehabilitation, also implementingtreatment and long-term management of patients with chronic respiratoryconditions within their homes.
- To undertake comprehensive respiratory assessments ofpatients requiring airway clearance, breathlessness management or those withworsening symptoms of long term condition.
- To identify and implement appropriate use of treatmentmodalities to support patients and their families to manage exacerbations andavoidance of hospital admissions, supporting self management.
- To act as a resource of specialist expertise for patientsand the wider MDT within Livewell Southwest, Primary and Secondary Carepartners.
- To be involved in the planning, co-ordination and deliveryof the Pulmonary Rehabilitation across the Livewell footprint includingdemonstration of exercise classes and provision of educational talks.
- Be familiar with OPEP devices / Salbutamol PGD /Ambulatory Oxygen provision via part B prescribing / Recognition of need forreferral to Oxygen clinic / Auscultation / Interpretation of x-rays /Understanding of Spirometry interpretation and Prescription of exercises.
All staff are expected to be able and willing to work acrossa 7 day service.

To ensure the service user is aware of the role andphilosophy of Respiratory Physiotherapy and Pulmonary Rehabilitation ,utilising all means necessary to enable individuals to understand the natureand purpose of proposed actions or intervention, including explaining anypossible risks involved.
To keep up to date with mental capacity legislation,assessing mental capacity and imparting sufficient information to the serviceuser in order to gain valid informed consent, which must be documented andgained at every intervention. For consent to be valid, HCPC stipulate it mustbe voluntary and informed, and the person giving consent must have the capacityto make the decision.
To act in a service users best interest at all times,factoring in their known views, beliefs, values, and instructions expressed,when undertaking care or an intervention where the service user lacks capacityto give expressed consent.
To consider if the lack of capacity may be temporary, andwhere possible, postponing intervention or decision making until they regaincapacity to ensure involvement of the service user in their own care as far asis reasonably practicable and possible.
To actively participate in complex MDT functions and supportindividuals with decision making around the application of the Mental CapacityAct, Deprivation of Liberty Safeguards and adhering to the principles of BestInterest legislation.
To undertake the comprehensive assessment of patients,including those with a complex presentation, using investigative and analyticalskills, and to formulate individualised management and treatment plans, usingclinical reasoning, and a wide range of treatment skills and options toformulate a programme of care and to support junior staff in this work.(Treatment modalities include; manual physiotherapy techniques, patienteducation, exercise classes, chest physiotherapy and other alternative options).
To demonstrate a high level of understanding of conditionsrelevant to the diagnostic area and those patients presenting with multipathology.
To demonstrate highly developed dexterity, co-ordination andpalpatory senses for assessment and manual treatment of patients wherenecessary.
Where necessary, to carry out baseline medical observationse.g. temperature, respirations and blood pressure checks. To record and sharethis information with other health and social care professionals, to aid themwith medical diagnoses and identification of most appropriate treatmentpathway.
To regularly record, act on and escalate as appropriate, thefindings of medical observations, e.g oxygen saturations, Heart Rate, BloodPressure, Respiration rate, Temperature. To assess if the appropriate responsemay require immediate treatment to resolve provided by the individual, orescalation to other services e.g GP, 111, 999
To have a working knowledge of the Care Act 2014 includingthe well-being principle, prevention, and the eligibility criteria.
As part of your role there may be a requirement /opportunityto identify the need for major and minor housing adaptation, and to refer tothe appropriate services for assessment.
To maintain competence in the use of Basic Patient Handlingtechniques and equipment including the prescription of basic mobility aids andadaptive aids for use in the community, including demonstration for patientsand carers.
To understand and comply with the safeguarding process andunderstand how and when alerts are required. To contribute to safeguardinginvestigations with support of other colleagues.
To co-ordinate, plan, record, review, report and discussgoal setting and treatment programme progress in close liaison with all membersof the multi-disciplinary team, peoples carers, and family in accordance withconsent given, agreeing assessment outcomes where appropriate.
To respect the individual values, cultural and religiousdiversity of patients and contribute to the provision of a service sensitive tothose needs.
To provide education and advice to service users, carersand/or family members regarding the impact of cognitive, perceptual, andphysical deficits on patient safety and independence in the home, leisure, andworkplace. This can include imparting sensitive information and advice toservice users and carers which could have life changing consequences.
To regularly undertake conversations regarding life limitingconditions, the role of palliative care services and treatments in the longterm management of chronic conditions, and supporting patients and theircarers/families with information and support when considering this.
To provide an opportunity for people and carers to expresstheir feelings whilst encouraging acceptance and adjustment to their newcircumstances.
To provide advice, signposting, guidance and information tohealth and social care professionals, patients, carers, relatives, or othernon-professional contacts.
This role may require you to facilitate the return of peoplecurrently in an inpatient setting/residential or nursing care placement whowish to return home.

To assist team managers to review incidents and support theimplementation of actions and learning for and with the team, this couldinclude supporting investigations of serious incidents and/or HR issues.
To participate in duty rotas and where appropriate, supportthe team lead with rostering.
To participate in MDT discussion within the CommunityRespiratory Team to explore new referrals, collaboratively triage andfacilitate the transfer of service users between the three services within theCommunity respiratory Service Provision.
To assist the team lead to respond to complaints andfeedback where appropriate.
To collect, collate and analyse informal and formal patientfeedback and implement changes within Pulmonary Rehabilitation

* Degree or equivalent in physiotherapy HCPC Registration Evidence of CPD maintained in a portfolio including attendance at recent postgraduate courses relevant to the clinical field Can demonstrate substantial relevant experience within the NHS, including experience in a range of experience e.g. orthopaedics, elderly care, medical, surgical, neurological and musculoskeletal conditions Can demonstrate substantial relevant experience in the assessment, treatment and management of patients with respiratory symptoms and conditions. Clinical educator qualification and experience.
* Ability to frequently travel independently and in a timely manner between Livewell Southwest sites and community locations including areas not served by public transport.
