Job overview

We are seeking an enthusiastic and clinically focused pharmacist to join the Clinical Pharmacy Team at Chesterfield Royal Hospital. This split role involves delivering high-quality ward-based clinical pharmacy services and teaching undergraduate pharmacy at the University of Nottingham.

The clinical component focuses on providing patient-centred pharmacy services at ward level, supporting safe and effective patient flow and transitions of care, and contributing to weekend pharmacy services as part of a rota.

The Teacher Practitioner component partners with the University of Nottingham, combining advanced clinical practice with involvement in pharmacy education and training.

Applicants should demonstrate strong clinical skills, effective communication, good time management, and the ability to work collaboratively.


Main duties of the job

Teacher Practitioner role working part time with the University of Nottingham delivering undergraduate teaching.

Provide dedicated ward pharmacy (DWP) services to a given ward across a range of medical and surgical specialties. DWPs key performance areas include: supporting ward rounds to influence clinical and cost‑effective prescribing; managing the process of discharge prescribing to improve patient flow and ensuring optimal safe transfer of care around medicines. You will also be supported by an innovative group of medicines management technicians.

The role will enable you to develop your clinical, leadership and management skills to the highest level. The DWP program works across a range of medical and surgical wards to provide broad clinical experience.

There are also opportunities to undertake clinical supervision and mentoring, service evaluations and audits to develop your skills and aid your career progression.


Detailed job description and main responsibilities

Job purpose

* The post holder will be responsible for providing clinical pharmacy services to patients.
* Oversee the use of medicines in terms of safety, efficacy, efficiency and economy.
* Identify and address medicines governance issues that arise from clinical service delivery.

Key Result Areas

* Provide clinical pharmacy services prioritizing discharge, medicines optimisation and clinical economy.
* Take a proactive approach to the management of ward discharges, to include prescribing of discharge medicines in line with local guidance.
* Work collaboratively with medical and nursing teams and Trust Operations Team to facilitate and expedite discharge.
* Ensure high quality patient counselling is delivered by the MDT during admission and at discharge.
* Support communication to GPs, community pharmacy and other primary care services.
* Undertake medicines optimisation activities in line with clinical specialty.
* Provide clinical pharmacy support to consultant ward rounds, MDT meetings and ward operational meetings.
* Work with prescribers and nursing teams to ensure high standards of medicines use including development and review of relevant guidelines.
* Review & investigate medicines incidents and liaise with the ward team to adopt changes in practice where necessary.
* Take a lead role in managing ward‑based medicines expenditure, including:
o Ensuring multidisciplinary ward team takes a thorough approach to obtaining patients' own medicines from home.
o Ensuring the ordering and supply of medicines receives appropriate scrutiny.
o Providing and presenting reports to the MDT, ward team and pharmacy in relation to medicines expenditure.
* Contribute to the pharmacy clinical economy group agenda.
* Undertake independent prescribing practice to patients within the specialty in accordance with agreed objectives/guidelines.
* Contribute to the delivery and development of clinical pharmacy services across the Trust.
* Actively participate within the multi‑disciplinary and clinical pharmacy team.
* Act as a source of expert advice on prescribing and medicines optimisation to medical and nursing staff within a defined clinical area.
* Provide supervision, training and tuition of pre-registration students and pharmacists, including tutoring for the MSc/Diploma in clinical pharmacy.
* Participate in clinical audit and other initiatives aimed at assuring high quality prescribing and pharmacy practice, within the context of Quality Governance.
* Provide training and education to other healthcare professionals.
* Provide professional supervision to technical and supporting staff and undertake the role of Responsible Pharmacist when required.

General Responsibilities

* Undertake any training (managerial and professional) identified by staff appraisal.
* Participate in on‑call duties, weekend and Bank Holiday working in accordance with agreed rotas and needs of the department.
* Attend work in case of disruptive incidents if fit and well in line with business continuity and major incident plans.
* Maintain confidentiality at all times relating to patients, staff and the Trust.
* Actively participate in the Staff Appraisal Process.
* Adhere to relevant Operational, Personnel, Health and Safety policies and procedures, including Fire, COSHH, No Smoking and Alcohol.
* Maintain a safe environment by adhering to local infection control policies.


Person specification


Qualifications and Training

* MPharm.
* Working towards Diploma/MSC Clinical Pharmacy.
* GPHC registration.
* Independent Prescriber registration.


Experience

* 18 months post registration in acute hospital pharmacy.
* Evidence of relevant CPD.
* Experience of influencing cost and quality of prescribing.
* Experience of service improvement.
* Experience of residency/on‑call.
* Experience of MDT working and ward rounds.
