An exciting opportunity to join the Hospital at Home team in Inverness. The service delivers comprehensive, patient-centred care at home to reduce hospital admissions and enhance patient outcomes. Funding has been secured to create this service for Inverness.
The specialist practitioners are supported by Senior Decision Makers and acute Consultants, working as part of the Integrated Team with AHP, Social Work and Care at Home colleagues as part of the Intermediate Care options available in Inverness.
Job Description: We are seeking a compassionate and skilled Hospital at Home Nurse to join our multidisciplinary team. In this role, you will provide acute care to patients in their homes, collaborating with doctors, specialists and other healthcare professionals to ensure high standards of care.
Key Responsibilities
* Deliver hospital-level nursing care to patients in their homes.
* Administer medications, perform diagnostic tests, and manage medical equipment.
* Collaborate with a multidisciplinary team to develop and implement individualized care plans.
* Monitor patient progress and adjust care plans as necessary.
* Educate patients and their families on managing health conditions at home.
* Maintain accurate and timely patient records.
Qualifications
* Registered Nurse (RN) with a valid nursing license in Scotland.
* Experience in acute care, emergency care, or home health nursing.
* Strong clinical skills and the ability to work independently.
* Excellent communication and interpersonal skills.
* Ability to travel to patients' homes within the designated area.
* Compassionate, patient-focused approach to care.
