Are you an experienced and motivated Advanced NursePractitioner (ANP) looking to make a real difference in primary care? We areseeking a dynamic and patient-focused ANP to join our friendly and progressiveteam at Millrise Medical Practice employed through our Primary Care Network(PCN) via the ARRS funding scheme.
27 hours per week working Monday, Tuesday and Friday
Main duties of the job
Key Responsibilities
* Conduct clinical assessments, diagnose, treat, and evaluate care for patients
* Independently manage a range of conditions, including acute and long-term illnesses
* Prescribe medications and treatments within scope of practice
* Lead on health promotion, disease prevention, and management of long-term conditions

Job responsibilities
Assessingand triaging patients, including same day triage
Asappropriate provide definitive treatment or make necessary referrals toprimary, secondary and social services within locally agreed pathways, guidanceand protocols
Manageundifferentiated undiagnosed condition and identify red flags/underlying seriouspathology and take appropriate action
Usecomplex decision making to inform the diagnosis, investigation and complete managementof episodes of care within a broad scope of practice
Ensurethat patients receive high quality care, delivered in a timely manner
Ensurecomplete and accurate documentation of each patient contact
Undertakehome visits for patients as required
Request and interpret investigations (e.g.pathology, imaging) and act on results appropriately.
Support management of long-term conditions inline with evidence-based practice and QOF requirements.
Person Specification
Qualifications
* Registered Nurse with valid NMC registration
* MSc or equivalent qualification in Advanced Clinical Practice (Level 7)
* Independent prescriber (V300)
