Community Nurse – Learning Disabilities – Band 6
An exciting opportunity has arisen for an experienced and highly motivated individual to join a community city-wide service within the Learning Disabilities Service. You will be an experienced and flexible individual with excellent communication skills and a passion for multi-disciplinary team working.
The role involves working within the framework of a multi-disciplinary team to provide support and guidance to children and adults with learning disabilities and their carers. This includes working within the framework of health facilitation (level one and two) and co-ordinating a clinical service under the supervision of Senior Community Nurses.
Key Responsibilities:
* To manage and co-ordinate a caseload under the supervision of the Senior Community Nurse.
* To focus on individual health outcomes for people with learning disabilities and offer support to access health services.
