Gtd healthcare is one of the largest NHS-commissioned Primary and Urgent care providers in North West England. We have an excellent opportunity for a Care Co-ordinator to work within the Oldham Bloods Service on a part-time basis.
As a Care Co-ordinator, you will be the first point of contact for healthcare professionals, patients, and their relatives accessing the bloods service within the Oldham area. Under the direction of the Senior Care Co-ordinator, you will register patients onto the relevant clinical system and book them directly into the most appropriate appointment.

Please note: The hours of work for this post are 21.5 hours per week, Monday - Friday, between 08:00 and 18:30.
Key Responsibilities:
1. Deliver a high-quality healthcare coordination service to patients and healthcare professionals.
2. Accurately record all relevant data on a range of clinical systems for each contact and direct appropriately.
3. Book appointments across multiple clinic locations.
4. Liaise with the Senior Care Co-ordinator regarding operational issues affecting patient bookings.
5. Coordinate follow-up care as directed by the Senior Care Co-ordinator.
6. Work with the multidisciplinary team to ensure the smooth running of the service.
7. Coordinate with GP Practices regarding patient referrals.
