Job Title: Band 6 CPN – Community Psychiatric NurseLocation: Grand Union Village, West London NHS TrustRate: £27 per hour Ltd (PAYE options available)Contract Type: Long-term NHS contractHours: Monday to Friday, Full-time (37.5 hours per week)Job Description: We are currently seeking an experienced Band 6 Community Psychiatric Nurse (CPN) to join a well-established NHS community mental health team at Grand Union Village, part of West London NHS Trust.This is an excellent opportunity for a CPN looking for a long-term assignment within a supportive and multidisciplinary environment. You will be working with adults with a range of mental health conditions, contributing to recovery-focused care plans, and managing a defined caseload under the CPA framework.Key Responsibilities:
* Manage and coordinate care for a caseload of service users with complex mental health needs.
* Conduct risk assessments, mental state examinations, and reviews under the CPA process.
* Deliver interventions including psychoeducation, relapse prevention, and medication management.
* Work in collaboration with psychiatrists, psychologists, social workers, and support staff.
* Provide crisis intervention and facilitate referrals to specialist services where required.
* Maintain accurate and timely clinical records using NHS systems (e.g., RiO).
Requirements:
* Registered Mental Health Nurse (RMN) with current NMC registration.
* Minimum 2 years post-registration experience in community mental health.
* Experience working at Band 6 level or equivalent in an NHS or community setting.
* Strong assessment, care planning, and risk management skills.
* Excellent communication and interpersonal abilities.
* Able to work autonomously and as part of a multidisciplinary team.
* Up-to-date PMVA and mandatory training desirable.
