Job Title: Social Worker - Young Adults Team Location: Worcestershire (Hybrid) Pay Rate: £33.30 per hour About the Role: Liquid Personnel is currently recruiting for a Social Worker to join it client's Young Adults Team at Worcestershire. What will your responsibilities be? In this role, you will support young people aged 16–25 with disabilities as part of the All Age Disability (AAD) Service’s Young Adults Team. You will assist in their transition from Children’s Services to Adult Services, focusing on education, employment, independent living, community inclusion, and health. The role involves working with individuals with Learning Disabilities, physical disabilities, or complex health needs. You will collaborate with Worcestershire Children’s Services, Adult Services, Special Educational Needs and Disabilities (SEND) teams, health professionals, and educational institutions, using Liquidlogic systems and applying both children’s and adults’ legislation to deliver person-centred support.
