Job Summary Kirklees Memory Assessment Service are seeking to recruit a Band 5 nurse to work within their team. This exciting opportunity is to work 37.5 hours per week with the Memory Assessment Service. This full-time post is based at Folly Hall in Huddersfield and the role will cover both North and South Kirklees. You would be working with a well-established multidisciplinary team that supports staff to develop both personally and professionally. We have a mix of Memory Nurses, Advanced Nurse Practitioners, Social Care Professionals and Occupational Therapists, with access to Dietetics and Psychology. A full comprehensive induction will be provided and access to further training options. Kirklees Memory Assessment Service has recently received their third MSNAP accreditation, which is endorsed by the CQC. The service has also received a Sustainable Mental Health Service Commendation. You will need to be able to work autonomously as well as within a team and be able to assess risks for people with dementia. All employees of the Trust are strongly encouraged to be fully vaccinated against COVID-19 to protect patients. Main duties of the job The postholder will work within the remit of the Band 5 job description which involves: Undertaking initial assessments of service users Intervention work, risk management and clinical decision making in line with leadership. Work in a collaborative manner with others to ensure seamless service delivery. Participate in the use of reflective practice, nursing research, and clinical supervision. The service places staff clinical and management supervision, appraisal, support and professional development as a high priority. Experience of working within the community would be advantageous. Full UK driving licence and use of own car is essential. To be competent in all aspects of care and risk management as defined within the framework of the care programme approach. To work collaboratively with clients, carers and multiple other agencies to provide effective care to optimize and maintain clients mental health and overall functioning, ensuring needs are met. Education Provide peer supervision and support to other team members and to take personal responsibility and supervise junior staff in the maintenance of written and electronic client and Trust records including inputting data related to own work activity.
