Main duties of the job
The post holder will be an experienced nurse with key practice nursing skills and competencies and should also have experience in managing patients with a long-term condition. Supported by senior nurses within the practice, you will deliver care within the boundaries of your role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. Experience of working in primary care essential.

1. RGN with current NMC registration
2. Evidence of Continued Professional Registration
3. Recognised qualifications in diabetes or chronic heart disease or respiratory medicine
4. A diploma in General Practice Nursing
5. Previous Experience in Primary Care
6. Qualifications/Certificates in Cytology, Travel vaccinations, Immunisations
