Key Responsibilities In-Home Assessments: Provide immediate assessments for patients discharged from hospital, helping them settle into their recovery at home. Multi-Agency Collaboration: Work closely with other healthcare professionals, conducting joint assessments and creating comprehensive, person-centred care plans. Safeguarding & Welfare: Ensure the welfare and safeguarding of vulnerable adults, managing complex cases and reducing risks. Care Act 2014: Perform Mental Capacity Assessments and implement the Care Act 2014, ensuring the best outcomes for service users. Engage with Service Users: Provide advice and support to service users, their carers, and professionals, enabling individuals to live safely and independently. Record Keeping & Data Management: Maintain accurate records and manage data in line with Council policies.
