Staff Call UK Ltd are recruiting for a Complex Care Coordinator to support the senior management team in leading safe, compliant and high-quality services across the Hull and East Riding Area.
This is a hands-on role supporting day to day service delivery, quality compliance, staff performances and service growth in our community based services.
Duties
Supporting management to delivery safe and effective services in line with regulatory and CQC standards.
Striving for excellence and promoting a positive culture for our service users, staff members and families.
Supporting with referrals and assessments, producing care plans and risk assessments and start up paperwork.
Creation and management of staff rota's ensuring appropriate cover is in place and ability to work under pressure at times, ensuring that all calls are covered.
Supporting clients, service users and families with regular visits, reviews, and case manage your client database.
Communicating with clients, families and healthcare professionals.
Ensuring care documentation and client reviews are accurate and updated.
Organising and participating in supervisions, appraisals and training.
Supporting and participating wider meetings with a multitude of professionals and representing the company to a high standard.
Participate in the \"On-Call\" rota providing out of hours decision-making and escalation of support.
Promoting the ethos and values of Staff Call and driving success and long term satisfaction for the companies clients.

Working in a background of mental health is a must for this role - understanding and working with Autism, LD, Personality Disorders and more absolutely essential.
The role is office based and around the HU13 area, however at times you will be required to travel for work. therefore candidates must hold a clean and valid driving license.
Working Hours Monday-Friday 0900-1700 with a 1 in 4 Weekend On Call Expectancy.
