The Great Yarmouth and Northern Villages PCH are looking for an enthusiastic nurse to join their busy team, which covers Great Yarmouth and the surrounding villages.
Main duties and responsibilities
Community nursing offers a different experience to traditional ward based nursing; it provides the opportunity to deliver a wide range of nursing interventions to people in their own home and within residential care. As a community nurse you will develop confidence and become expert in clinical skills including wound care and have the opportunity to deliver end of life care for people in their preferred place.
The community nursing service is commissioned to provide a 24/7 service. The core hours are between 08:00 - 16:30. Early and late shift patterns are also part of the working week. Applicants will be required to participate in working other shifts as part of the role including weekend and bank holiday working on a rotational basis.

Responsibilities (condensed)
Contribute to, support, and promote ECCH’s strategic direction, values, and culture in relation to community health services.
Discuss all treatment options with sensitivity and expertise. Act as a patient advocate, respecting patient confidentiality and diverse backgrounds.
Act as a resource for other health and social care professionals on matters relating to community nursing. Participate in clinical supervision and in learning opportunities within the clinical area.
Develop internal and external networks to promote Primary Care Home. Contribute to clinical governance through clinical risk assessment and clinical audit.
Support a positive working environment, collaborate to deliver high quality clinical services, and contribute to change and innovation within Primary Care Home.
Work as part of the multidisciplinary Primary Care Home to deliver planned and unplanned services and develop care pathways for patients with diverse healthcare needs. Review delegated caseload with senior team members to ensure high quality care.
Promote patient self-care and independence using health coaching techniques. Collaborate with residential care colleagues to promote high standards of care.
Contribute to reviewing current policies/protocols/guidelines and developing clinical pathways and outcomes for proactive and reactive services. Plan and organise complex clinical activities across community settings to deliver best practice.
Maintain own professional development and ensure compliance with ECCH risk management, health and safety, and policy procedures. Ensure accurate patient records in line with ECCH and NMC standards. Manage resources and equipment cost-effectively and contribute to training for patients and carers to maintain independence.

Qualifications
* Professional Registration with NMC Registered Nurse Level 1 (Adult)
* BSc in Nursing Practice or equivalent.
Desirable
* Mentorship/preceptorship qualification ENB 998; evidence of working towards a nursing degree.
Experience
* Evidence of continued professional development; ability to work unsupervised; clinical knowledge within scope of practice; previous healthcare experience.
* Desirable: recent clinical experience in community nursing; experience of multidisciplinary team working.
