As a Senior Community Nurse/District Nurse, you’ll be at the heart of delivering patient-centred care. Your role will involve:
* Admission Avoidance: Implementing proactive care strategies, such as participating in MDTs, caseload reviews, and complex patient reviews/visits, to prevent unnecessary hospital admissions.
* Health and Independence Promotion: Empowering patients to manage their health conditions effectively at home, utilising personalised care plans and supporting the NHS's commitment to enhancing out-of-hospital care.
* Leadership and Development: supporting in leading a dedicated community nursing team within a multi-professional environment, fostering a culture of continuous learning and improvement.
We’re looking for a motivated Registered Nurse with a graduate qualification in a health-related subject and post-registration community experience, or someone with the Specialist Practitioner Qualification in District Nursing. You’ll play a vital role in leading a dedicated community nursing team within a multi-professional setting, where your contributions are valued, and your development is prioritised.
Main duties of the job
* To Practice as a senior member of the community nursing team, working within the Integrated Community Services (ICS)
* To provide a case management approach to care for an identified number of patients working closely with GPs, the ICS and other Statutory and Voluntary agencies.
* To provide advice, information and education to patients and carers with the aim of enabling patients to enjoy maximum independence, health and wellbeing.
* To creatively manage the holistic and clinical needs of patients with acute, sub-acute and long-term conditions.
* To enable patients living long term physical health challenges to live well within their communities, with a focus on not only their physical health needs but also their wider holistic needs, e.g. mental health, learning disabilities, psychological needs, social needs and spiritual needs
