We’re looking for a Care Coordinator/Carer to join our brilliant team in Reading. This crucial role ensures our clients receive exceptional care. The role involves organising schedules based on clients' requested call times and care professional availability, as well as maintaining effective communication with staff, clients, families, and professionals alongside delivering care directly. This role is currently a 50% service based role, you would be expected to cover care calls for half the week (M-F) and 50% of the role is care coordination. What will you do? · Producing weekly rotas and care scheduling to ensure adequate provision of care · Undertake regular care visits to support our clients (all private clients) · Daily troubleshooting and problem solving · Managing staff absence, allocating extra calls, and cancelling care calls when necessary · Assisting in the training and induction of new team members · Facilitating Good Oaks in providing a continuous and reliable service Who are you?
