Overview

Advanced Clinical Pharmacist - Bank. You will join us on a bank basis to develop and deliver the medicines optimisation and safety agenda alongside a team of clinical pharmacists. Key responsibilities include repeat prescribing, managing long-term conditions, conducting clinical medication reviews, and clinical audit and safety work. The role requires expertise in medication management and patient care.


What you will be doing

* Processing repeat and non-repeat medication requests
* Clinical medication reviews with patients to optimise their treatment
* Support the development and implementation of prescribing policies
* Reconcile medicines after hospital discharge and ensure patients receive required treatments
* Monitor and review prescribing patterns to ensure compliance with the Pan Mersey Formulary
* Answer medicine-related queries from healthcare staff and patients
* Contribute to audits and quality improvement initiatives
* Provide clinical advice
* Support public health campaigns, including immunisations and flu vaccination initiatives
* Ensure compliance with CQC standards and health, safety, and infection control policies
* Work collaboratively with the wider healthcare team, including GPs, nurses, and other professionals, to deliver exceptional patient care
* Advocate for patients, ensuring clear communication about their treatment options
* Contribute to fostering a supportive and efficient team environment


What I need

* Be an independent prescriber
* A minimum of 2 years post-qualification experience
* A minimum of 2 years experience of working in general practice
* Excellent written and verbal communication skills
* Ability to work with minimal supervision remotely, with excellent organisation skills
