Part Time - Social Prescriber (Link Worker)
Central and South Primary Care Network is looking to recruit an experienced Part Time Social Prescribing Link Worker (20 Hours Per week only) to join our multidisciplinary team.
This is an exciting opportunity to make a real difference to patients’ lives by helping them take greater control of their health and wellbeing. You will connect people to community-based support and services that address non-medical issues affecting their health, such as loneliness, housing, employment, debt, and lifestyle.
This role is funded through the Additional Roles Reimbursement Scheme (ARRS) and forms a vital part of our commitment to personalised care and improving population health outcomes across Knowsley.
Salary and Hours
* 20 hours per week, worked flexibly over 3 or 4 days
Main Duties of the Job
* Support patients referred by GPs and practice teams to identify personal goals and develop individualised wellbeing plans.
* Help people access appropriate community, voluntary, and social enterprise (VCSE) services.
* Work closely with GP practices, the wider PCN team, and local partners to ensure coordinated care.
* Maintain accurate records and contribute to practice systems to ensure continuity and evaluation of support provided.
* Build and maintain strong relationships with community organisations to expand local support options.
* Promote equality, inclusion, and health equity in all work.

Key Responsibilities
* Provide personalised support to individuals, enabling them to identify their goals and co‑produce a personalised plan to achieve these.
* Act as a key point of contact for patients, offering continuity and coordination of care.
* Support patients referred by GPs, practice staff, and other healthcare professionals to access non‑clinical services and community resources.
* Work with individuals to build confidence, resilience, and independence by identifying issues affecting their health and wellbeing.
* Collaborate with GPs and multidisciplinary team to identify patients who would benefit from social prescribing interventions.
* Contribute to clinical meetings and case discussions to provide insights into non‑clinical solutions for patients.
* Maintain accurate and timely records of referrals, interventions, and outcomes, ensuring integration with the practice’s clinical systems.
* Develop strong links with local voluntary, community, and social enterprise (VCSE) organisations to expand referral options and resources available to patients.
* Work collaboratively with local stakeholders to map and maintain an up‑to‑date directory of services.
* Identify gaps in service provision and work with the PCN to address these.
* Apply a holistic approach to patient care, considering physical, emotional, and social needs in line with ARRS guidance and NHS England’s personalised care model.
* Promote health equity by supporting patients from diverse backgrounds, including those from vulnerable or marginalised groups.
* Adhere to relevant legislation, including data protection, safeguarding, and equality and diversity standards.
Person Specification
Qualifications and Experience
* Experience in a health, social care, or community-based role (essential).
* Awareness of the NHS Long Term Plan and ARRS framework (desirable).
* Relevant qualification in health and social care, public health, or counselling (desirable).
* Minimum Level 3 qualification relevant to the Social Prescribing Link Worker role (essential).
* Training or certification in health, social care, or counselling (desirable).

Ability to travel across the Central and South PCN area as required.

Salary
£30,349.64 a year (pro rata for 20 hours per week, worked over 3 or 4 days)
