Utilise various Discharge to Assess and re-ablement pathways to support individuals to maximise their independence away from the hospital setting, with a focus on discharge to the right place at the right time. Responsible for working with people to assess both the immediate and ongoing needs, in order to support a person in implementing their longer term care and support needs.
