The Independent Living Service has 30 staff made up of Occupational Therapists, Disability Officer and Community Assessment Officers and includes the Sensory Team and Telecare Team. The aim of the posts are to aid with discharge facilitation, supporting people to remain independent at home and community follow up. The posts will sit alongside Community Therapy and the Neighborhood team and looks to support people using a strength-based approach to maximise independence. The Physiotherapy role will involve supporting timely hospital discharges and reducing readmission risks through community-based support. You ll conduct Completion Care Act 2014 assessments, including support planning and risk assessments. Work collaboratively with Community Therapy and Social Work teams is key. Acting as a trusted assessor for equipment provision and moving and handling support, as well as promoting independence by linking individuals to reablement, universal, and voluntary services will be vital in this role. This post offers an opportunity to contribute to the ongoing development of innovative ways of working across health and social care in order to improve the experience of service users and carers and to promote wellbeing and independence. The ability to develop, maintain and improve relationships with service users and carers is essential in this role as well as the attributes required to participate in inter-disciplinary team working.
