Job overview
Are you a band 6 or an experienced band 5 looking for an exciting new opportunity? We are seeking a skilled, motivated, and dynamic Nurse with a passion for high quality care and service development.
You will be responsible for managing your own clinical caseload of community patients well as supporting the HCA/rehabilitation assistants with training, supervision, and joint sessions.
The Rehabilitation And Frailty Team (RAFT) provides daily input for up to 21 days in the patient’s own home. The service hours are 8am – 8pm with a care element added 7am – 10pm 365 days a year. You will be working occasional weekends and bank holidays on a rota basis.
Main duties of the job
We continuously prioritise our caseload to be responsive to the changing daily needs of our service users, communicating closely with other service providers to avoid admission where it is safe to do so and providing onward referrals to other community services including health and social care. As part of the Rehabilitation And Frailty Team we see a wide variety of patients including neurological, orthopaedics, falls, frail, end of life, admission avoidance, long term conditions, and musculoskeletal allowing staff to get broad experience across different specialities. You will be part of a friendly and approachable team who work well together and will be well supported by your senior management team.

Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop in sessions focusing in engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
High Cost Area Supplement – Fringe
This post also attracts payment for High-Cost Area Supplement of 5% of the basic salary (minimum payment of £1,303 and a maximum payment of £2,198).
Detailed job description and main responsibilities
The post holder will be a Registered Nurse (RN12-NMC) who will have significant intermediate and community care experience at Band 5/6. The post holder will be responsible for leading nursing care for a designated caseload. The post holder will be professionally and legally accountable for all aspects of care relating to clinical areas. Responsible for patients allocated to them and will assess, manage, and deliver care to meet the complex needs for those patients requiring intermediate care and primary care services. The post holder will be able to manage and support their peers, working on their own initiative and but also an effective team member along with the requirements of pre-registration students. They will be committed to working collaboratively, actively supporting, and liaising with other health, social care professionals and other agencies. Due to the service provision, there will be an expectation of the ability for seven days working within this role.
