Rehabilitation Assistant – Community Stroke Rehabilitation Team

Integrated Community Stroke Rehabilitation Team in Barnsley offers an opportunity to work as part of a multi‑disciplinary team of nursing and allied health professionals in both community and inpatient settings, carrying out designated duties competently.
Essential requirements: ability to use initiative, good communication skills and effective time management.
Service provides care to patients following a stroke in both inpatient and community settings, enabling patients to access rehabilitation therapy upon discharge home.
Vehicle use to undertake necessary travel as part of the post is essential.
Applicants must prove the right to work in the UK or alternative visa routes, as sponsorship is not available.
Job Responsibilities
Key duties of the role include:
* Work as part of a multi‑professional team providing safe, compassionate planned care.
* Carry out rehabilitation programmes set by therapists and nursing teams.
* Assist clinical staff with patient observations in inpatient units.
* Support and motivate patients to participate in rehabilitation.
* Participate in training, education and clinical supervision.
* Work both as part of a team and independently in the community.
* Contribute to care planning and service‑user safety.
* Maintain positive therapeutic relationships and provide appropriate adjustments.
* Use appropriate equipment and handling techniques.
* Assist with de‑escalation, restraint or seclusion when necessary.
* Attend to personal care needs of service users.
* Participate in flexible rostering, providing 24‑hour/7‑day care when required.

Experience
* Providing personal care and procedures for clients with health/social needs.
* Working in a caring environment at a band 2 or equivalent level.
* Dealing with the public.
* Working as an effective team member.
* Independent or multidisciplinary working ability.
* Initiative, decision‑making and prioritisation.
* Commitment to user/carer involvement.
Qualifications
* GCSE A‑D or grade 3–9 in Maths and English or equivalent.
* Apprenticeship level 3 or equivalent (RQF NVQ level 3) in Health & Social Care or equivalent.
* Or Level 2 qualification with ability to demonstrate competencies at Level 3.
* Care Certificate or achievement within 12 weeks of commencement.
* Level 4 units in Health & Social Care.
* Further study towards Level 4 qualification (willingness).
