As our Lead Reablement Worker, you'll be at the heart of supporting our customers aged 55+ to stay independent in their own homes. You'll lead our amazing team of Reablement Workers, helping them deliver person-centred care that makes a real difference. From supporting people with daily living to coordinating care plans and working with health professionals, you'll be key to helping our customers live well and stay connected to their community.
Typical day as a Lead Reablement Worker
Supporting customers with varying needs, including dementia and frailty, to regain or maintain independence in their daily activities.
Developing and reviewing personalised care plans that focus on reablement, safety and the customer's aspirations.
Providing guidance and leadership to Reablement Workers, ensuring high-quality, person-centred care is delivered.
Managing more complex personal care needs, including stoma and catheter care, while promoting customer choice and dignity.
Working closely with external professionals, like health and social care teams, to make sure customers get the best support possible.
