Lakeside Healthcare is a large GP partnership that provides NHS care for around 170,000 patients across Cambridgeshire, Peterborough, Northamptonshire and Lincolnshire. We are made of eight GP practices across 14 sites that are part of their local communities.

About the Practice/Department/Team
Lakeside Healthcare is a new type of NHS general practice which builds upon and celebrates all that is great about traditional primary care services. We are local surgeries run by doctors, nurses and associated staff who know their patients and care deeply about the quality of health provided in their communities. We build upon this by investing in people, facilities, and equipment to provide services that are more accessible, more convenient, and more diverse within the local community. We work for our patients.
The nursing team is a well-established team with positive effective teamwork and team collaboration. This increases efficiency in patient care resulting in better patient experiences.
Job Introduction
The role of the PCN Community Nurse Practitioner is to work with the lead GP in developing and providing a proactive and responsive community based primary care service for patients based in the community with acute minor illness, long-term conditions and complex case management of patients who are prone to hospital admission. Some key areas of the role are to develop and review admission avoidance care plans as well as contributing to and working with the practice team and Specialist Neighbourhood Nurse; in actively managing the patient's health.
What you will do
* Reviewing and providing care for patients in the community, whether for pre‑planned services or same‑day urgent care.
* Diagnosing and treating patients presenting with minor illness.
* Managing ongoing medical and nursing needs of patients, including advice and follow‑up care.
* Prioritizing patient needs appropriately, including arranging investigations and making referrals when necessary.
* Any patient assessed as requiring secondary care referral should be discussed with the registered GP before the referral is made.
Essential Criteria
* RGN Qualification (Adult Branch or relevant)
* NMC 1st level Registration
* Evidence of continued professional development.
* Experience of working at level 6; ideally in community nursing or with transferable skills. Working autonomously in an independent role as well as in a team.
* Experience of providing emergency care for patients and management of patients with complex needs
