Clinical Assessor - GP/Community Line

The closing date is 13 June 2025

This is a fixed term contract ending in March 2026.

There is an exciting opportunity for a an experience Band 6 nurse who has the confidence in assessing remotely, insight of Integrated Front Door Pathways and community services. The GP/Community Phoneline is dedicated to patient flow and supporting patients to remain well in their own home, which is key to managing flow and maintaining essential to planned and unplanned care.


Main duties of the job

The GP/Community Phoneline is an 8am - 8pm service 7 days, with the aim of supporting patients to remain well in their own place of residence where it is clinically safe to do so, reducing the number of unnecessary A&E attendances and unplanned admissions. A significant proportion of the workload involves frail elderly patients and those with palliative care needs both in a domestic setting or care facility.

This development post will give the applicant an opportunity to gain experience working within Integrated Front Door Footprint and to spend time working in an area alongside Advanced Clinical Practitioners within Same Day Emergency Care & Medical Assessment Unit.

Job responsibilities

The post involves working with other services including community services, primary care, social care and other localities pathways both in the acute and community, inclusive of NHS@HOME. We are looking for individuals who are able to work with patients to provide personalised care, even when that means the easiest pathway cant always be followed. The post holder needs to be able to work with an MDT approach and hold respect and regard for all health and care professionals involved in complex patient care. The applicant needs to be comfortable with problem solving. The applicant needs to be strong patient advocates and demonstrate a strong drive for service improvement keeping patients at the centre of their motivation. The service will continue to develop and the applicant will need a creative approach to work and be receptive to constant change.


Person Specification


Qualifications

* Willingness to take Non-Medical Prescribing qualification
* Willingness to work towards completion of physical assessment/clinical reasoning module
* Current NMC/ HCPC registration
* Diploma in a relevant health care topic/working towards ENB 998 / C & G 730 /SLiP Educational qual.
* Physical assessment/clinical reasoning module (PACR)
* Pathophysiology & Diagnostic reasoning for Advanced Practice (PDRAP)
* Remote Clinical Decision Making


Experience

* Experience in more than one of acute, community, primary care, or palliative care settings


Knowledge & Skills

* Extensive clinical knowledge in the care and management of the frail elderly, those with palliative care needs, acute and chronically sick patients within and outside hospital
* Demonstrates self-learning to improve knowledge and skill base
* Computer Literate
* Knowledge of National Health Service agenda in relation to Acute Care, Clinical Governance, and the relevant clinical professional agenda
* Understanding of and ability to implement, evidence-based practice.
* Knowledge of risk management
* Care Home Management experience
* Palliative End of Life Experience
* Telephone triage Experience
