You will provide health education and practical support to women and their families, working with the midwives within the team toprovide personalised care and support to the women and families within the teams caseload.

Detailed job description and main responsibilities
1. You will work with the midwives within the team toprovide personalised care and support to the women and families within the teams caseload.
2. You will act as a link between maternity and allied health services when required by signposting and referring to agencies under the supervision of the named midwife within the continuity of care team.
3. You will contribute to service development and are responsible for self-development. You will be part of a team based at Royal Preston Hospital/Chorley and South Ribble Hospital working flexibly within the hospital and community setting covering various shift patterns. You will be expected to demonstrate self-directed learning, practice and reflection.
4. You will participate in leading parent education programmes with a focus on the promotion of normality and increase confidence in early parenting skills, including infant feeding practices.
5. You will completed documentation to enable quality data collection, audit and analysis as part of the Enhanced Continuity of Care teams' service/quality improvement agenda.
6. There will be a period of competency-based training both initially and over the first year of employment / promotion which you must complete in order to perform all the duties of the job to the required standard.
7. You will be proactive in supporting public health promotion advice and promoting services.

Qualifications & Education
Essential criteria
8. Level 3 Health & Social Care (or equivalent).
9. 5 GCSE’s Grade C/5 including English and Maths or equivalent qualification
10. Care Certificate
11. Willingness to complete Level 4 module / diploma-level training maternity‑specific academic development within 24months
12. Evidence of MSW competencies
Desirable criteria
13. Completion of a Level 4 MSW programme, maternity-specific module, or Level 4/5 apprenticeship in maternity care.
14. Formal antenatal education, public health promotion or coaching
Knowledge & Experience
Essential criteria
15. Demonstrated understanding of continuity of care models, personalised maternity pathways and multidisciplinary coordination.
16. Experience supporting women and families with complex social, emotional or clinical needs, with appropriate escalation.
17. Experience delivering or supporting public‑health interventions (e.g., infant feeding, safe sleep, smoking cessation)
18. Minimum 12 months’ experience as a Band 3 MSW or equivalent, demonstrating competent clinical practice.

20. Experience supporting and guiding junior staff or students
