Advanced Clinical Practitioners PHL Group have an exciting opportunity for experienced Advanced Clinical Practitioners to join us in both our Out of Hours service and triage. As an Advanced Clinical Practitioner, you will be accountable for delivering safe patient care to a high standard. You will need to be confident in working independently and autonomously at an advanced level of practice across our services, including remote telephone triage, home visits and face-to-face consultations in a primary or urgent care clinic. Acting within your professional boundaries, your duties will include all aspects of the patient journey including initial history taking, clinical assessment, diagnosis and treatment, prescribing medication, and evaluation of the patients care. Advanced practice will include evidence of working within the four pillars: clinical, education, research and leadership. Location There are a variety of opportunities across the county in our clinics, cars and our contact centre based in Whiteley. Hours Example: Mid-week 19:00 23:00 or 23:59 07:59 Weekends: 08:00 -15:59, 10:00 -15:59, 15:00 22:59, 18:30 23:59, 23:59-07:59 Rate of pay has added enhancements when working out of hours. Advanced Clinical Practitioners (Nurse or Paramedic) Key Responsibilities: Assess undifferentiated patients (with no restrictions on age ranges from babies to adults) with a range of acute, non-acute and chronic medical conditions. Take history, make appropriate physical examinations, formulate differential diagnoses and formulate/manage treatment plans when working in any clinical environment. Prescribe medication as required within your clinical competence. Recognise the need for and initiate emergency treatment to save a life or to safeguard the interests of the patient and refer to emergency services when appropriate. Request investigations and be able to interpret pathology and radiology results to ensure appropriate treatment and/or onward referrals. Advanced Clinical Practitioners (Nurse or Paramedic) Key Skills and Experience: 3 years of experience working in a primary care GP practice. 3 years experience in an urgent care setting such as an Urgent Treatment Centre or Emergency Department. At least 6 years post qualification. Excellent knowledge and experience in the assessment and management of minor injury and illness presentations with evidence of competency completion for role. Registered with the NMC/HCPC Non-medical prescriber Completion of MSc Advanced Practice Pathway or equivalent.
