Acute / Enhanced Clinical Practitioner - Acute Hub, Martock, Somerset

Symphony Healthcare Services has an opportunity for an experienced Acute / Enhanced Clinical Practitioner to join our passionate, multidisciplinary teams across the South Somerset West (SSW) Primary Care Network (PNC) GP practices.

This role will be based at the Acute Hub in Martock Surgery, but you will be expected to work flexibly across all practices within the SSW PCN as and when required. These include:

Martock Surgery

South Petherton Surgery

Hamdon Medical Centre (Stoke Sub Hamdon)

Crewkerne Medical Centre

The hours available are 27 hours per week. Working pattern (days and hours will be discussed at interview.

The annual salary for this post is £49,256 per annum (pro rata for part time workers) and the option to join either a NHS or NEST pension, as one of our many employee benefits.


Main duties of the job

Working within their professional boundaries, you will provide care for acutely presenting patients including history taking, clinical assessment, diagnosis, treatment and coordination of any necessary follow-up.

You will demonstrate safe, clinical decision-making with a strong evidence basis, for patients within the same-day urgent care setting, in accordance with published clinical pathways, policies and protocols.

Job responsibilities

You will bean experienced nurse/practitioner who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.

You will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills at an advanced level, for patients within the general practice.

You will also will demonstrate critical thinking in the clinical decision-making process and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures. In order to work at this level, NMC requirements for advanced practice must be met.

Person Specification


Qualifications

* Qualified paramedic or specialist paramedic registered with the HCPC
* GCSE or equivalent in English and Mathematics
* A Level qualifications or equivalent
* Diploma or Degree in a healthcare related discipline


Knowledge and Skills

* Excellent interpersonal and communication skills
* Knowledge of primary care operating environment
* Excellent analytical skills, including analysis of data
* Time management skills and the ability to prioritise workload
* Demonstrable commitment to and focus on quality and safety
* Able to work outside of normal hours as and when required
* Ability to collaborate constructively with internal and external partners to create the conditions for successful partnership working
* Understanding of the South Somerset Symphony Programme and New Models of Care
* Understanding of the NHS GP Contract
* Use of EMIS searches and protocols
* Able to multi-task and cope with an unpredictable work pattern


Experience

* A minimum of 5 years paramedic work experience
* Evidence of appropriate continuing professional development activity
* Ability to assess and manage patient risk effectively and safely
* Experience of working within a clinical / nursing team
* Experience of dealing with vulnerable patients
* Experience of working in primary care
* Experience of working in a GP practice
