We are looking for anexperienced, enthusiastic nurse to join our team, working 2 nights a week out of 7. During the hours of 8pm and8am they will travel to patients across North Derbyshire to provide palliativenursing care.
Main duties of the job
Purpose
The Roaming Car Service is annight service covering the hours of 8pm to 8am. During those hours the RoamingCar Service Nurse and a Health Care Assistant (HCA) will travel to patientsacross North Derbyshire, planned and unplanned, to provide palliative nursingcare.

Job responsibilities
Key Tasks
To work with, and support,the patient, relatives, and carers; being sensitive to their changing physical,psychosocial and spiritual needs.
To provide skilled nursingcare within the care plan of the District Nurse, and in accordance with themedical care prescribed by the GP.
To provide nursing care andsupport to the families and carers of individual patients in the place theycall home across North Derbyshire.
To communicate and liaisewith the patients usual care team including District Nurses, other palliativecare support such as the DHU/111 Roaming Service as well as the Hospices RoamingCar Service Manager or deputy, reporting on the condition and needs of thepatients and providing and maintaining nursing reports and records, asnecessary.
To collect and report anyrequired data to support delivery of the service, documenting directly on to SystmOneor in reporting systems as appropriate.
To attend team meetings ona regular basis, undertaking clinical supervision, mandatory and on-goingtraining and education as required.
To maintain professionalconfidentiality in all matters relating to patient care.
To be professionallyaccountable as described in the NMC Code of professional conduct.
Clinical
To beresponsible for the administration of controlled medicines and be aware ofcurrent drugs/therapies used in symptom control.
Demonstratenursing procedures, where appropriate, to carers or colleagues to assist inpromoting and maintaining patient comfort.
To be competentin the use of syringe drivers, other specialist equipment and aids, and utilisethese when appropriate.
To be competentin all clinical procedures required for symptom management in end-of-life care,following Blythe House Hospice policies and procedures
Advise theHospice at Home team of any equipment and aids required for the provision ofefficient nursing care to meet the needs of the patient.
To be familiarwith the procedure in the event of an emergency or death of the patient,including that for the verification of death.
To conduct theverification of death, if formally trained/qualified and competent to do soonce it has been confirmed to verify through contacting DHU overnight.
To be aware ifa DNACPR/ReSPECT order is signed and in place for each patient and takeappropriate action when required. Also, to alert the co-ordination team if aDNACPR/ReSPECT Form is not in place.
To develop,with support where required, a high level of specialist palliative careknowledge which includes key issues relating to the last year of life, such as:

To assist the RoamingCar Service Manager in the delivery of the service as directed to ensure that the service isefficiently and effectively managed.

To understandfinancial implications of service delivery.
Quality and Education
To contributeto the delivery of ongoing training programmes to ensure the Roaming CarService team and HCAs have up to date knowledge and skills relevant to theirroles and responsibilities.
To ensure andrecord compliance with CQC regulations and contribute to provide evidence ofcompliance.
To ensure allsignificant events, drug errors or service issues are reported in a timely way.
To ensureissues are raised of alleged wrongdoing or malpractice with the appropriateline manager as well as ensuring that any member of staff who raises suchissues will be protected under the Hospice Whistleblowing policy.
To beresponsible for accurate recording of incidents, accidents and complaints inline with Blythe House Hospice policies and procedures.
To contributeto the further development of methods of data collection in line with nationalguidance and Blythe House Hospice policies.
To ensure allconfidential information, hard and soft data within the department is managedsecurely and maintained in accordance with GDPR, relevant policies andprocedures.
To support theprocess of ensuring the quality of care is measured against agreed standards.
To inputappropriate data/outcomes using audit tools when required.
To support themonitoring and evaluation of the performance of the Hospice at Home service.
