Were looking for someone to work on in our Hospital Discharge team. Youll be working closely with integrated discharge hub staff to support customers to be discharged from hospital and those who need support to stay safe and well in their own homes in the community. Please note We will reimburse mileage and parking expenses for any travel undertaken as part of your role. Job requirements Youll complete tasks to help customer get home safely from hospital such as doing property inspections, collecting belonging, dropping off food parcels, or arranging quotes for deep cleans so discharge can go ahead. Youll do ad hoc or regular visits to people identified in the community as needing further support and complete tasks as requested. Examples of tasks might be checking homes are warm and safe, checking medication food and other essentials are available. You will be visiting wards, engaging with customers and sourcing volunteering services to support the customers wants/needs. You will collaborate with other teams within the hospital to ensure a timely discharge. This is a physically active role, where you can expect to walk approximately 5-6 miles per day while delivering small food parcels. What were looking for: Excellent communication skills. An aptitude for working with vulnerable and/or older people. Understanding of what great customer service is. Ability to work under pressure and manage high caseloads. Excellent ICT and administrative skills. Identifying issues to help prevent delays in discharge. Hold a full UK driving license with use of your own vehicle including business insurance.
