An exciting new opportunity has arisen for a Primary Care Network (PCN) Health & Social Care Coordinator to help deliver personalised health and care as set out by the Personalised Care Institute and based at a GP Practice.
There are between hours available for this role. It will be based at Woolton Hill Surgery, a GP practice split over two sites and part of the West Berkshire Rural PCN.
Our ideal candidates will proactively identify and work with a cohort of patients to deliver personalised care, particularly ensuring Care Plans are regularly reviewed and updated. They will also assist patients with interventions to support their health and wellbeing. Excellent personal, co-ordinational and communication skills are required for this role, which seeks to holistically bring together a person's identified care and support needs.
This interesting and varied patient-facing role will work alongside our Social Prescribing Link Worker. Under GP supervision, the Health and Care Coordinator will also work with other primary care professionals within the PCN and locality.
One of the key responsibilities will be to identify and manage a caseload of patients, co-ordinating their care packages and, as appropriate, refer patients back to other health professionals across health, social care and mental health as appropriate.
An important part of the role will also be the preparation of evidence and data, as well as organising and attending monthly multi-disciplinary (MDT) meetings. Primary Care experience not essential.
We welcome applications from Health Care Assistants, or persons with a qualification in health or social care allied profession, or experience in a supporting role at a GP Practice. Our expectations and qualifications for this role are:
Proven 1-year (minimum) experience in a health or social care profession in the UK together with an understanding of Primary Care, how it works and the services available. Our ideal candidates will be team players with the ability to work proactively and using their own initiative.
Competent IT skills, attention to detail and a good understanding of managing data and tracking for audits.
Confident in dealing with a wide range of stakeholders (e.g. GPs, Patients, Carers, Community Nurses, Healthcare professionals) and organising Multi-Disciplinary Team meetings as well as working proactively alone and as part of a team.
