An exciting opportunity has arisen for an enthusiastic and motivated Social Worker to join the Amber Valley Short Term Assessment and Reablement Team.
The Short Term Assessment and Reablement Team provides a multi-disciplinary approach to assess, support and enable adults to maximise their independence and to assess for any potential long term care needs. We are committed to working in a person centred and strength based way, recognising people's assets and strengths by enabling them to identify and achieve their goals. The team are community based, but will also support people leaving residential and nursing establishments, and will include facilitating safe and timely discharges from some hospital settings.
We are continuing to offer a hybrid approach where our team are currently working from home and from the office, based on the needs of the service and policy requirements.
The Successful applicant will be able to demonstrate the necessary skills, knowledge and experience to undertake this role in order to provide a safe, accountable, cost effective and efficient service to our clients which helps them achieve their outcomes and aspirations. You will need to demonstrate your ability to carry out social care assessments and support plans in line with the Care Act 2014 and the Mental Capacity Act 2005. We expect that you will be confident making decisions and managing a busy case load with good, clear communication and working in partnership.
You will be part of a duty rota and will be expected to undertake safeguarding enquiries in line with the Care Act 2014. Please see the job and person profile for further information.

Experience of working in Adult Care is preferred but not essential. The post is for a 5 in 7 working pattern but at the present time, staff are expected to work on a Monday to Friday basis. Prior notice will be given in advance if this changes.
