Key Responsibilities
* Assess adult care and support needs and develop detailed, outcome-focused care plans
* Implement and regularly review support plans to ensure needs continue to be met
* Undertake adult safeguarding enquiries and contribute to safeguarding plans
* Provide guidance and advice to colleagues and partner agencies on safeguarding matters
* Promote independence using recognised social work methods, models and tools
* Make sound professional decisions in line with current legislation and policy frameworks
* Work collaboratively with service users, families and partner organisations
