Overview
An exciting opportunity to join our Intensive Services team, supporting the delivery of person-centred care services across East Renfrewshire and enabling individuals to remain safe and supported within their own home.
You will have a good understanding of health and social care, the positive impact of timely discharge from acute settings and the value of outcomes‑focussed supports for people to enable them to return to and remain in their communities. You will recognise the value of building and maintaining effective relationships with other health and care professionals and agencies.
Responsibilities
* Undertake assessment and review of service provision to service users.
* Support the referral pathway for access to care at home services from hospital and community settings.
* Assist individuals to continue living in their own homes by arranging care services and providing appropriate advice and support.
Requirements and Qualifications
* Estimated to be Regulated Work with Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. It is an offence to apply if barred from working with protected adults.
* If you have lived or worked outside the UK in the past 10 years, there may be a requirement for an overseas police check.
