An exciting opportunity has arisen for an enthusiastic Social Worker to join the East Place Home First Hub Team, working within the NHSE Hospital Discharge Service: Policy & Operating Model to ensure a safe and timely discharge from hospital and swift follow up assessments, once the person has been discharged. Norfolk County Council are on an ambitious journey and having made great strides over the last few years ,we now need to take forward, further rapid redesign in line with national policy and quality improvements to ensure adults being discharged from the James Paget University Hospital and community short term placements, are supported to return home, access reablement opportunities and plan their ongoing care and support needs once discharged. The Homefirst Team are working towards an integrated care system, alongside East Place; Primary Care Hubs and Occupational Therapists. The successful applicant will provide support with follow up work in East Place, whilst also provide in-reach support with the Acute Hospital undertaking transfers of care within the Virtual Homefirst Hub with our partners. They will complete various assessments, reviews and Safeguarding Vulnerable Adults enquiries.
