A rare and exciting opportunity for a band 6 community nurse has arisen. We are seeking an enthusiastic and highly motivated nurse to join our dynamic multi- disciplinary service.
The post would suit an experienced nurse seeking a new challenge or a newly qualified nurse as a developmental role.
The successful applicant will be joining an innovative team of nurses providing high quality heath interventions as part of the wider forward thinking, diverse multi-disciplinary team, working collaboratively with nurses and other professional clinicians as well as multiple agencies.
You will be able to demonstrate that you are able to apply your nursing skills to a wide range of situations, working with a range of adults with a learning disability living within the community. You will be responsible for managing a caseload across the geographical area of East Surrey, working closely with other agencies as well as members of the multi-disciplinary team.
There is an emphasis on the importance of clinical supervision and continued professional development, which we regard as a vital part of the delivery of care. You will be part of an innovative and dynamic team which utilises an evidence-based perspective. We will support and encourage you to develop specialist experience and expertise as appropriate
Main duties of the job
1. To provide specialist health support, advice and guidance to individuals, families and carers.
2. To provide specialist support as part of the multi-disciplinary team.
3. To complete specialist health assessments as clinically indicated.
4. To participate in referral meeting, team meetings and wider MDT meetings.
5. To contribute to team and service development.
6. To carry an individual caseload.
7. To keep up-to-date with research and development in your fields of practice.
8. To attend training relevant to the role.
9. To maintain NMC registration and remain up-to-date with research and development in learning disability nursing practice.
10. To complete individual risk assessments under supervision and to put a safety plan in place if needed.
11. To communicate with and support social workers, care providers and other agencies and services.
12. To supervise and assess student nurses on placement with the team.

Detailed job description and main responsibilities
29. To practice within the NMC Code of Professional Conduct.
30. To be professionally accountable for the effective and efficient management of own caseload, taking into account the need to combine one’s own responsibilities and those of the multi-disciplinary team.
31. To be responsible for the setting of the highest standards of care within the resources available and to review and monitor these on a continuous basis through the process of audit, including case review, on-going supervision, and appraisals of self /others where relevant.
32. To lead and encourage a positive approach to health education and health promotion.
33. Offer consultancy and specialist input to a range of other professionals and agencies in relation to clients with complex needs over a range of complex situations, which will include complex healthcare needs and personal relationships.
34. To be actively and regularly involved in working parties/forums where policy and service changes are proposed that will have an impact on other disciplines, departments and agencies, this will include primary, secondary and tertiary caring services.
35. To participate in ensuring that accommodation, equipment and consumable goods are efficiently and effectively requisitioned, safely used, disposed and replaced.
36. To participate in the investigation of complaints, concerns or incidents related to nursing services in accordance with the level of experience; and encourage comments and suggestions on improving the quality of the service in liaison with the person who has continuing responsibility.
37. To be actively involved in the recruitment and selection of staff in conjunction with the person who has continuing responsibility in liaison with the Team Manager/Service Manager.
38. Advice and influence local and regional policy, strategy and operational practice in relation to health needs of people with learning disabilities.
39. To undertake generic and/or Health and Safety risk assessments as appropriate, and report findings to the Team Manager/Professional Leads and be involved in adverse incident reporting.
