Key Responsibilities / What You Will Do Deliver expert occupational therapy assessments and interventions within the community. Work closely with the admission avoidance team to support early intervention and prevent hospital admission. Utilise your A&E experience to provide responsive, informed support for urgent or emergency presentations. Collaborate effectively with multidisciplinary teams to ensure holistic patient care. Drive to patient homes to deliver individualised, goal-focused therapy in community settings. Requirements / Candidate Criteria Essential HCPC-registered Occupational Therapist. Previous experience in A&E and community settings. Ability to work independently across community environments. Strong assessment, clinical reasoning, and documentation skills. Access to a vehicle for community visits. Desirable Experience in admission avoidance or reablement services. Familiarity with community care pathways and integrated care systems.
