Grade C- £35,412 - £41,771

Golden Hello payment - £1,000 and £5,000 per annum; see below for further details and conditions.

Working 36.5 hours per week

Discharge to Assess - Acute Hospital Teams (Good Hope Hospital and Heartlands Hospital)

An exciting opportunity has arisen to join the Acute Hospital Sector Discharge to Assess Teams, based within the Good Hope Hospital and Heartlands Hospital.


Responsibilities

The successful candidate will work across busy Multi-Disciplinary Teams to facilitate social work practices, and may be required to support other teams within the Discharge to Assess Service, as appropriate.

A key aspect of the role is supporting and strengthening multi-disciplinary working (MDT), fostering collaborative and effective working practices within a fast-paced environment.

You will work as part of the Discharge to Assess Social Work Teams to deliver on person-centred practices where citizens are enabled to exercise choice, control, independence, and build resilience with a focus on prevention and early intervention to support, guidance and sign-posting in addition to assessments, via the 3 Conversations model, in accordance with the Care Act 2014.

You will have demonstrable social work experience, the ability to work positively as part of a Multi-Disciplinary Team, in partnership with other stakeholders, together with an interest in and skills to work within the specific Discharge to Assess Multi-Disciplinary Team.

Working within the legislative framework, you will carry out social work functions, utilising relevant legislation to complete conversations and provide support and guidance, drawing on sound interpersonal and communication skills. You will require the ability to produce reports and the resilience to deliver under pressure within required timeframes.

You should be eligible to be registered with Social Work England. We are also excited to extend this opportunity to Social Work Apprentices and Students who have confirmation of their qualification (although is awaiting actual certificate) after completion of their studies this coming year.


Qualifications

Eligible to be registered with Social Work England.

Experience in social work within a health or community setting.

Demonstrable ability to work effectively within a Multi-Disciplinary Team.

Strong interpersonal and communication skills, with the resilience to deliver within required timeframes.

Knowledge of the Care Act 2014 and related legislation.
