An exciting and rare opportunity has arisen for an experienced mental health nurse to join the Leeds Assertive Outreach Teams (AOT).
Responsibilities
* Work as a member of the Assertive Outreach team providing a city‑wide service for patients with highly complex, severe and persistent mental health problems seven days per week.
* Provide day‑to‑day coordination of the team as required.
* Act as care coordinator responsible for the management of a caseload, including assessment of care needs, development, implementation and evaluation of care packages.
* Ensure an effective learning environment for students and other staff, regardless of professional background.
* Carry continuing responsibility for setting and monitoring standards of care in line with Clinical Governance requirements.
* Promote nursing values and interventions within the multidisciplinary team and with service users.
* Establish and maintain good working relationships with other community teams/agencies within the statutory, voluntary and private sector.
Qualifications and Experience
* Preferred experience of working in community care and care coordination, though not essential; support will be provided to gain confidence.
* Experience working with psychosis and a genuine interest in supporting the recovery of people with severe and enduring complex mental health problems.
* Ability to engage difficult‑to‑reach service users, including those with a dual diagnosis and a history of risk.
* Knowledge of working within the Mental Health Act.
* Good communication and team‑working skills; ability to deliver person‑centred care within a multidisciplinary team.
Working Hours and Travel
The team provides a city‑wide seven‑day extended‑hours service: Monday‑Friday 9 am‑8 pm, weekends and bank holidays 9 am‑5 pm. Due to the area covered, a car driver is essential.
